In order to complete the Vital Statistics regarding your life, here is a list of the information you will need:

Please provide the service provider with a copy of your "Designation of Bodily Remains Agent" that our office has prepared for you.   This will help them with all of the information and put together an estimate based on what your stated wishes are.

If you travel frequently and especially internationally, I strongly recommend investing in a comprehensive travel insurance policy that will include repatriation of your body to a U.S. Funeral Home or cover the cost of your cremation and return of your cremains.  Most policies will also include the cost of obtaining death certificates from the foreign jurisdiction, as well as coordination with the U.S. State Department for "Report of Death of U.S. Citizen Abroad" which will likely be necessary to obtain for your estate administration.
